Your pet is very important to us!! At WAGZ grooming studio we would like to assure you that every effort will be made to make your pets grooming experience as safe and enjoyable as possible. Please carefully read each paragraph of this form and sign to complete.
* Occasionally, grooming can expose a hidden medical problem or aggravate a current one. This can occur during or after the groom. This includes allergies and infected ears. Please let us know if your pet has sensitivities with soaps, shampoos or perfumes. We have a Hypo-allergenic shampoo on hand for any pets with sensitivities. We will not be held responsible in the event your pet has an allergic reaction. If you notice your pet is itchy when it goes home please notify us immediately so we can add to your pets notes. Please let us know if your pet has or you suspect it has an ear infection.
* Although accidents are very rare, there is a high risk when dealing with animals. Animals can make quick, sudden movements, grooming equipment is sharp and although we use extreme caution and care in all situations, possible problems could occur such as cuts, nicks, scratches, quicking of the nails etc. In the event that an accident does occur you will be notified. If we find the accident needs immediate vet care we will contact you about taking your pet to its regular vet or emergency vet. If accident/injury occurs due to matting, temperament/difficulty of the pet or self mutilation, vet expenses will be the responsibility of the owner.
All dogs must be picked up immediately following their grooming service.
* We recommend that your pet be up to date on vaccines, on flea and tick treatment and having prevention from Bordetella (kennel cough) . If we find any fleas on your pet it must be bathed in our natural flea shampoo at an additional cost. If you do not want a flea bath for your pet then it must be immediately picked up and we can reschedule when you have taken care of the flea problem. You will be responsible for paying for 50% of your appointment cost. Please inform us before your appointment if you suspect that your pet may have fleas so we can schedule your pet accordingly.
* In the event that your pet becomes too stressed or becomes dangerous to groom, we will stop the grooming process and have the pet picked up immediately. We have the right to refuse, stop, or cancel the grooming service at any time . You will be charged based on the services performed. In the event your pet is trying to bite the groomer or the tools a muzzle will be used.
* We require 24 hours notice to cancel your grooming appointment. Any missed appointment/last minute cancellations will be subject to a 50% cost of services fee. Please allow us enough time to fill the appointment. Continuous missed appointments will result in having you prepay for your service or termination from our salon.
* It is extremely important that you bring your pet at its scheduled appointment time and that you pick up at the time given. If you are going to be late please notify us immediately as we may have to reschedule your appointment. One late client can throw the entire day off schedule. Your pet MUST be picked up immediately after its groom. We do not have room to hold your pet in our facility and it will disrupt the next grooming appointment and the groomers schedule. We ask that you please do not book personal appointments at the same time as your grooming appointment. There is a 30 minute window from when we call/text you to pick up your pet, if you are late picking your pet up a "late pick up fee" will be charged of $10 for every 15 minutes late after the 30 minute window.
* We will not cause serious or undue stress to your pet by dematting. If your pet is severely matted the groomer may have to take it short. The groomer will discuss with you your options and will also educate on proper tools and techniques that should be used for your pets specific coat type. We recommend your pet to be on a 4-8 week grooming schedule to avoid matting.
If for any reason you are not satisfied with your grooming service performed, please let us know within 24 hours of your dog coming home and allow us the chance to resolve the issue or give explanation.
Our groomers like to take photos/videos of their work (your dog) and showcase it on social media platforms. By signing this agreement you agree that our groomers can use your dogs photo/video on social media.
Please understand that pricing can fluctuate based on the condition of your dogs coat or if you change styles/length.
All policies are put in place for the safety and comfort of your pet and our groomers.

I have reviewed this Service Contract for accuracy and understand the contents of this contract. I affirm that I am the rightful legal owner of the pet(s) for which services are being rendered. I authorize this signed contract to be valid approval for future grooming services, permitted the grooming salon and WAGZ grooming studio to accept telephone reservations for service without additional signed contract or written authorization. I understand that pricing is subject to change. This agreement is valid for all pets that I currently own or will own in the future.
I, the undersigned, hereby acknowledge that my pet, as listed above, has significant matting and/or tangles in its fur. I understand that this matting can cause discomfort, pain, and even injury to my pet during the grooming process.
I understand that the grooming salon cannot be held responsible for any injuries or adverse reactions caused by the matting, and I release them from any liability. Removing matts can result in bruising of the skin, abrasions, nicked warts/cysts that were covered or clipper irritation. Hematoma's can also develop in severely matted ears. Which are sacks of blood that fill up at the tips of the ears when matting is removed. Blood will then seep through the skin which will result in which looks like nicks on the ear tips. Vet attention may be required for this. I also understand that my pet may require additional services, including but not limited to dematting, which will result in additional charges of $30-$50 which will be determined at the end of the groom.
If the matting is too severe for us to safely remove or the dogs behaviour will not allow us to do it we will refer you to your vet for a sedated matt removal where it can be comfortably done for your dog.
Matted dogs will need to be booked as the last appointment of the day to ensure safety.
Our salon will remove matting ONE TIME on your pet. We encourage our clients to keep their dogs coat in a healthy condition whether it being maintained at home or by us, the professionals. We will require you to stay on a regular schedule with your dog to prevent matting from happening or unfortunately we will have to dismiss you as a client. We want your dogs appointment with us to be as enjoyable as possible so we can build a life-long trusting relationship with them.
I authorize the grooming salon to take any necessary actions to ensure the safety and well-being of my pet during the grooming process. I understand that if my pet's matting is too severe, the grooming salon may be unable to safely remove the mats and may need to shave the affected area or the entire coat.
Please understand that in severe cases of matting your dog may act abnormal after its groom. Examples of this may include shaking/trembling, hiding, loss of appetitive, consistent head shaking (which may lead to hematomas), licking and itchiness.
I have read and understood the contents of this matted release form and agree to its terms and conditions. I certify that I am the legal owner or agent of the owner of the pet named above and have the authority to execute this release.
